Include the following documentation when you send your tax package:
The Tax Orgainizer worksheet is a tool to use in gathering your tax docs. When completing the Organizer, you do not have to fill out every section. Use it as a guide for gathering the documentation that you need. For your W-2 and 1099 info, you do not need to fill out the details on the organizer. Just check off that you have the ones listed from previous years or cross out items that you no longer have. Feel free to write notes on it to give details or to ask questions.
Clarification: there are two documents/worksheets to complete and submit:
1. “Questions worksheet” is the 3 pages of yes/no questions plus the Charitable contribution log for those who itemize deductions.
2. Tax Organizer is the 3-5 page doc where you enter your current year data. The first page has your contact information. Please check and update this page. These worksheets shows your figures from last year. You will enter your 2024 figures in the “this year” column
When sending your tax package, include:
- Questions worksheet doc- answer each question and sign. Include documentation for each question that you answered “yes”. Complete the charitable contribution log if itemizing deductions.
- Completed and signed Tax Organizer worksheet –
- W-2s
- 1099-R, 1099-INT, 1099-DIV, 1099-B, 1099- MISC, 1099-NEC, 1099-K, 1099-Q
- For any bank or investment account that you did not receive a 1099-int or 1099-div, include the December statement that shows year-to-date interest.
- 1098 for each mortgage
- Settlement Statement if you bought, sold or refinanced a house
- 1098-T for tuition, documentation for payment of all education expenses, and the accounting statement from the university that shows tuition and fees billed and paid
- 1098-E for student loan interest
- 5498SE for HSA account
- 1099SE for HSA account
- Completed Charitable contribution log
- Due diligence Questionnaire for Child Tax Credit, Earned income credit, and American Opportunity credit and Head of Household Status.
- If you received any correspondence from the IRS or your state during the year, please include the letter if you have not already given it to me
